What it costs, before you book
Every price the practice charges is all in and includes applicable state tax. There is no separate consultation fee, no membership, and nothing added at the appointment. Where a state charges its own fee, it is set out below.

These fees are paid to the state, not to this practice, and they are not included in the prices above.
- Georgia$30 for the card, plus a $3.75 service fee if you pay through the state's online portal. Paying by money order avoids the service fee. The card is valid for five years, but the physician certification behind it has to be renewed every year.
- TexasNothing. Texas does not require patients to register or pay a state fee, and issues no patient card.

What the appointment price includes
- A full appointment with Tiffanni Forbes, MD herself, not a questionnaire review
- Phone or video, whichever you prefer
- Her time for your questions during the visit
- Direct contact with her afterwards if something comes up
- A support team for technical help and state process questions
- Applicable state tax, already in the price
What it does not include
The state fees above, and the cost of any product itself. This practice does not sell cannabis products, does not price them, and does not refer patients to any specific retailer.
How payment works
Payment is handled inside the secure patient portal when you book, under the portal's own terms and consents. This website does not take payment details and does not store them.
A physician evaluation cannot promise certification or approval by any state registry. Certification is at Dr. Forbes's independent medical judgment, and approval decisions rest with the state.
